Anyone wishing to have their child baptized at Holy Cross Parish must be registered as a parish member and complete the following:

Parents must have taken the pre-baptism class.

Sent in the "Godparent Certificate of Eligibility and Intent" to the Holy Cross Parish office.
Provided a copy of the child's birth certificate to the Holy Cross Parish Office.

Once all these requirements are met, then parents may fill out the form below.
